Open Enrollment

 

Yearly, the Board of Trustees for the Teamsters Local 631 Security Fund gives you the opportunity to change your medical plan between the Trust plan and the Trust’s HMO’s.  This annual open enrollment will be your only opportunity to change your Medical plan coverage this year. 

You may select either the Trust Indemnity Plan or Health Plan of Nevada (HPN) or PacifiCare. Health Plan of Nevada (HPN) and PacifiCare are the two medical HMO options made available by the Trust.   

Enclosed for your review is the Benefit Summary Comparison of the medical plans being offered in this Open Enrollment period.  This benefit summary has been prepared for your convenience for comparison purposes only.  Please refer to the detailed Summary Plan Description (SPD) benefit booklet for a complete description of benefits.  This comparison should also be used instead of pages 45 – 47 of your SPD.

If you do not wish to change Plans at this time, do nothing.  However, if you wish to make a change or wish to receive an entire packet containing more detailed information about any of the plans being offered, and enrollment forms for medical changes, you can either call the Administrator’s office and request that one be sent or stop in at their office to pick one up. Forms must be completed and returned to the Administrator’s office no later than Monday, June 26, 2006.  Plan change forms, completed and returned by June 26, 2006 will be processed and the new coverage will be effective July 1, 2006. 

Please consider your options carefully as this is your ONLY OPPORTUNITY to change medical coverage until next year at this time.  REMEMBER – A new medical enrollment form only needs to be completed if you opt to change plans.  Once you have made a change in coverage, it will remain in effect until next year’s re-enrollment effective date.

 

Updated COBRA Rates

 

Every year, the Board of Trustees evaluates the cost of the Trust’s benefits and makes adjustments to COBRA rates.  The new rates will be effective June 1, 2006. The following table contains the old and updated rates:

The Administrator will advise you of any shortage due you through a separate letter if you have paid COBRA contributions for the months of June, July or August 2006 at the old rate.

 

Worker’s Compensation Claims

 

If you have been injured on the job, you have an obligation to file a worker’s compensation report with your Employer.  Your Trust will not pay for any work related injuries and will deny all claims related to work injuries.  If your company is disputing the claim, you can then file an appeal with the State Board of Appeals to have your worker’s compensation claim reviewed.  Only those claims that have been rejected by the State Board of Appeal will be considered for payment by the Trust. 

 

Did you know…?

 

Did you know that if you get a divorce, but fail to tell your Administrator that you are divorced, and claims are paid for your ex-spouse, upon discovery you WILL HAVE TO REIMBURSE THE TRUST for all claims related to your ex-spouse from the date your divorce became final.  To avoid this, be sure to inform the Administrator of the divorce and bring a copy of the decree into the Administrator’s office. 

It is important that the administrative office has your correct address, to keep you updated about your benefits.  If this notice was forwarded to you from an old address, please contact the administrator’s office to report your address change as soon as possible.  Also, please make sure your life insurance beneficiary is up to date.  If not, please complete a new beneficiary form at the Administrators Office.
