New Pre-Authorization
Provider
- I.D. Card Update
The following plan changes have a direct impact on
all Indemnity Plan Participants. We urge you to read this notice carefully
and familiarize yourself with the changes and updated material.
Beech Street Corporation
Effective February 1, 2005,
the Pre-Authorization/Utilization Review
Organization changes from Healthcare Strategies to
Beech Street Corporation.
Beginning
February 1, 2005, you and
your physician must contact Beech
Street Corporation at 1-800-407-1228
to obtain the required medical review
pre-authorization for all hospital admissions, all outpatient surgeries and
for all diagnostic tests and laboratory work exceeding $500.
Please note: IF
Medically necessary services are provided without prior authorization, your
benefits will be reduced to 50% of covered charges.
No benefits will be provided for any days of
confinement or services or supplies determined to be not medically necessary
by the medical utilization review organization.
Identification Cards
As a participant under the Indemnity Plan, you
are responsible for knowing your health benefits and for following certain
procedures in order to be eligible for the maximum benefits payable. To
assist you, we are in the process of printing new I.D. cards updated with
all the appropriate telephone numbers you will need to obtain pre
authorizations, and any other information you may need concerning your plan
of benefits. In the meantime, remove the HealthCare Strategies sticker from
the back of your current I.D. card to expose the Beech Street
pre-authorization telephone number.
When you receive your new I.D. card,
please destroy your old ones.
You
should carry your I.D. card with you at all times and present it whenever
you obtain medical services or require hospitalization.
