The Board of Trustees for the
Teamsters Local 631 Security Fund is continuing to enhance your benefits by
giving you more choices for your medical and dental plans. During this annual
open enrollment, as a Plan Participant, you will have the only yearly
opportunity to change
Medical and Dental
plan coverage.
For Medical, you may select
either the Self-Funded Indemnity Plan
or Health Plan of Nevada (HPN)
or PacifiCare. Health Plan of Nevada (HPN) and PacifiCare or the two
medical HMO options available from the Fund.
For Dental, you may select
either the Self-Funded Indemnity Dental Plan, which gives you the opportunity to
use DDS (Diversified Dental Services) as a PPO provider or any dentist of your
choice or Nevada Pacific
Dental, a new Dental HMO. This new Dental HMO works in a similar manner to a
Medical HMO. Your co-pays for Dental Services are reduced under the Dental HMO,
however you must use a Dental Provider in the Nevada Pacific Dental HMO
Network.
Enclosed for your review are Benefit Summary Comparisons of
the medical and dental plans being offered in this Open Enrollment period.
These benefit summaries have been prepared for your convenience for comparison
purposes only. Please refer to the detailed Summary Plan Description or benefit
booklet of the appropriate plan for a complete description of benefits.
If you do not wish to change Plans at this time, you need
do nothing. However, if you wish to make a change or wish to receive an entire
packet containing more detailed information about any of the plans being
offered, and enrollment forms for medical or dental changes, you can either call
the Administrator’s office and request that one be sent or stop in at their
office to pick one up.
Please
consider your options carefully as this will be a great opportunity to change
your medical and dental coverage. REMEMBER – A new medical and/or dental form
only needs to be completed if you opt to change plans, and that this will be
your only opportunity to change medical or dental coverage until the
middle of 2003.
Forms must be completed and returned to the Administrator’s office no later than
Friday, June 21, 2002. Plan change forms, completed and returned by June 21st
will be processed and the new coverage will be effective July 1, 2002.
Changing a beneficiary on your health records does not
constitute a change on your pension or union death benefits. You should always
remember to keep that current as well.

Fair
Southwest Administrators will be at the Health Fair on June
1, 2002 at the Local to assist you in your Health & Welfare questios and any
related Open Enrollment and Pension issues. The Health Fair is from 9:00 am –
1:00 pm.
It is important that the administrative
office has your correct address, to keep you updated about your benefit plans.
If this notice was sent to a wrong address, you must contact the administrator’s
office to report your address change as soon as possible.
