Important Notice

 

 

The Board of Trustees announces the following important changes to the Teamsters Local 631 Security Fund for Southern Nevada plan,

Effective August 15, 2002

 

Medical Plan Exclusion

The Plan Exclusion for intentional self-inflicted injury has been deleted. 

 

Dental Plan Changes

Certain aspects of the dental plan are changed: 

Periodontal scaling – Limited to a depth of 4mm or more and payment allowed for each quadrant once per year only.  Allow 1/2 benefit when 4 or less teeth are in a quadrant.

Periodontal Maintenance – Limited to one treatment per calendar quarter if the patient has a history of prior scaling and root planing.

Periodontal Chemo Chips – Allow charges up to a maximum of $300 per calendar year and eliminates the current limit of two (2) sites per case.

Debridement – Limited to new patients only.

Direct and Indirect Pulp Caps – Limited to teeth where decay radiographically encroaches on the pulp of the tooth.

Bone and tissue grafting - Now covered under the plan only in conjunction with placing implants.

Mayo Clinic

The Mayo Clinic facilities are not included in the Beech Street PPO network.  Due to increased fees that Mayo intends to charge the member and the Fund, and the availability of other equally qualified facilities, the Trustees chose to eliminate the Mayo Clinics from the network.  This includes the Mayo facilities in Scottsdale, AZ, Rochester, MN and Jacksonville, FL.  Claims for charges incurred at any Mayo Clinic on or after August 15, 2002 will be handled as Non-PPO claims.  Contact Beech Street or the Administrative Office for Centers of Excellence in the PPO network.

 

Please note that Walgreens is back as a part of the Fund’s nationwide pharmacy network with Express Scripts effective July 1, 2002

 

Reminder:  Update Your Mailing Address

As always, the Trustees want to be certain all plan participants receive updates to their health and welfare plan;  therefore, make it a priority to keep the Administrative Office informed of your current mailing address.  You need only stop by the Administrative Office and complete a new enrollment card or your Union Office and fill out a change of address form.
